What you’ll do as a Program Specialist at UCP:

  • Evaluate referrals to determine appropriateness of placement and work collaboratively with the Residential Manager and the Residential Director to assess the ability to support individuals referred to residential services.
  • Develop annual Personal Support Plans that meet the specific needs and desires of each individual supported in collaboration with all team members. 
  • Monitor the implementation and the effectiveness of Personal Support Plans/Individuals support plans. 
  • Provide the assistance, training, and support necessary to assist the individual to develop and increase personal competencies and become socially integrated into their communities.
  • Conduct weekly audits of files of residents on caseload. Communicate results of audits to Residential Manager, Residential Supervisor and the Residential Services Director.
  • Develop and implement new policies and/or procedures within the functional area to support the continuous improvement of operational effectiveness and efficiency.
  • Complete all assessments and quarterly reports within the required timeframes. Ensure compliance with applicable licensing regulations.

Specific Requirements and Preferences:
At a minimum, you’ll need:

  • Must be at least 18 years of age.
  • High School Diploma or General Education Development (GED) Certificate required.
  • Associate’s degree or technical certification within a related area required. Bachelor's degree within a related area preferred.
  • 2 to 5 years of previous job related experience required.
  • 5 to 7 years of previous job related experience preferred.
  • A valid driver’s license with a clean record.
  • Access to regular use of a vehicle with valid insurance coverage.
  • Flexible to work all assigned work schedules including holidays, evenings, nights, and weekend hours as business needs dictate.
  • Ability to read and interpret documents in English. Ability to read and communicate verbally in English. Written communication skills in English may also be required.

It’d be great if you also have:

  • Regulatory Compliance (ie. Chapter 51, 6400).
  • Knowledge specific to the needs of individuals with intellectual and developmental disabilities, autism, and/or the aging population.
  • Multi-Site Management.
  • Developmental Planning and Goal Setting.
  • Knowledge of disability services funded through Medicaid and through PA’s Department of Health and Human Services.
