LPN Consultant/Educator

Position Description:

$1,000.00 Sign-on Bonus for applicants hired between 12/16/22 and 4/3/2023. Paid upon completion of 90 days of employment
Completing Quarterly Medical History Reviews
Attending staff and individual support plan meetings
Attending hospital/rehab discharge meetings
Providing staff training and education
Triaging calls outside of normal working hours
Ensuring the health, safety, and welfare of individuals with a primary diagnosis of MH/IDD

Specific Requirements and Preferences:

Full-time, salaried exempt position
Community based medical oversight and staff education
Must be licensed as a practical nurse in the State of Pennsylvania
Minimum of 3 years post-licensure experience required
Basic computer skills
Must possess a valid driver’s license and at least 3 years of good driving experience with your license, and must meet our driver eligibility criteria
