Providers who received an error message when attempting to submit final expenditures on May 31, 2025, may do so through midnight, Tuesday, June 3, 2025.

As a reminder, only ODP providers with approved ARPA Grant applications should use the following survey to request final reimbursement for expenses incurred. ODP is no longer accepting new applications.
